A billable FQHC visit is a medically necessary, face-to-face encounter between a patient and a qualifying FQHC practitioner. Under the prospective payment system, that single encounter — not each individual service — is what generates the PPS payment, which is why understanding what makes a billable FQHC visit is central to capturing the revenue you’ve earned.
Who and what qualifies
The encounter must be face-to-face with a qualifying provider. Medicare recognizes physicians, nurse practitioners, physician assistants, certified nurse-midwives, and certain behavioral health practitioners (clinical psychologists, clinical social workers, and others) as FQHC practitioners whose visits can anchor a billable encounter. A nurse-only or administrative contact generally does not qualify on its own.
Crucially, the PPS payment is bundled. Most services and supplies furnished during the same encounter are included in the single per-visit rate and cannot be billed separately to Medicare. Lab tests (other than venipuncture) and certain preventive services follow their own rules, but the default is one bundled payment per qualifying visit.
Where billable FQHC visit revenue gets lost
Two opposite mistakes cost money. Billing an encounter that doesn’t meet the face-to-face or qualifying-provider standard leads to denials and compliance exposure. But the more common loss is the reverse: failing to recognize and bill a visit that genuinely qualified, or not attaching billable procedures to the qualifying visit’s claim so their charges are captured.
