When a second same-day visit is payable
Medicare recognizes limited exceptions where two encounters can be billed on the same day. The most common are a medical visit and a mental health visit furnished the same day, and a situation where, after the first visit, the patient suffers an illness or injury requiring an additional, separate visit. In these cases, each qualifying encounter can be paid, provided the documentation clearly supports two distinct visits.
- Medical plus mental health — a qualifying medical visit and a qualifying behavioral health visit on the same day.
- Subsequent illness or injury — a new problem after the first visit that requires separate care.
Outside these exceptions, FQHC same-day billing stays one encounter per day. The revenue risk runs both ways: missing a legitimate second encounter leaves money on the table, while billing two where only one qualifies invites denials and audit attention. Clean documentation that distinguishes the two visits is what makes the exception defensible.
